We offer two convenience options to submit your registration, medical and dental information prior to your visit. This will ensure that we have everything ready for you and can start with you right on time the day of your visit. Please allow yourself 20 minutes to complete the data. For your convenience, you can do this online at our secure center or print the PDF forms (listed on the left side of this page) and fax to 206-720-0400 prior to your appointment.



From your first contact with the prestigious Advanced Cosmetic & Laser Dentistry you will feel the difference. Everybody does. At your first visit you can expect:

- An in-person interview with your treatment coordinator,

- In depth review of your records,

- A private discussion of your dental “wish list”.

Generally, this whole process can take two or three office visits. Your new patient experience consists of a records appointment, a comprehensive exam, all necessary x-rays, and your first hygiene visit.

Consultation:

Many people are not aware of the various treatment options available to them when it comes to their dental health. It is for this reason Advanced Cosmetic & Laser Dentistry offers Complimentary Consultations. At your private consultation you have the opportunity to ask questions, learn about all your treatment options, and interview Dr. McKay and his staff. The fit should be a good one.
